The Indiana Department of Health requires that the following immunization records be on file with the University Health Center for all full-time students:

• MMR – two doses required

It is recommended that you have a physical and get your health information form completed by orientation (ACE or SOAR) to allow time for the Health Center to ensure that all required information is complete. SOAR: Student Orientation and Academic Registration (SOAR) is a comprehensive orientation designed for all new incoming students and their families.
